Physicals we do

School physicals. Starting at a Florida school for the first time? Florida asks for a health exam done within one year before enrollment. We fill out the Florida School Entry Health Exam form (DH 3040).

Sports physicals. For school teams and rec leagues. We fill out the FHSAA physical form (EL2) used by Florida middle and high school sports. It’s good for 365 days from the exam date.

Camp physicals. Bring your camp’s health form and we’ll fill it out.

Immigration physicals. Call for details on immigration physicals.

What happens at the visit

Dr. Uriel starts by going over your child’s health history and your family’s health history with you. Then he examines your child. He checks:

  • Heart and lungs
  • Ears, nose, and throat
  • Vision
  • Reflexes and flexibility
  • Posture, joints, and strength

Then he fills out the forms you brought.

A sports physical doesn’t replace your child’s yearly checkup. If a checkup is due, ask about it when you call.

What to bring

  • Your school, sports, or camp forms. Fill in the parent sections at home if you can.
  • Your child’s vaccine record
  • Your child’s insurance card
  • A photo ID for the parent or guardian
  • Glasses, contacts, or a mouth guard or retainer if your child uses one

Who we see

We see newborns, kids, teens, and young adults, generally up to age 21. If your child is close to 21, call and we’ll confirm we can do the visit. We’re accepting new patients.

Which forms do you fill out?

The Florida School Entry Health Exam (DH 3040), the FHSAA sports physical form (EL2), and camp or league forms. If your school or team uses a different form, bring it and we’ll fill it out.
